The Lost or Stolen US Passport application process is for:
- when the applicant is currently age 16 & over AND
- the Lost or Stolen US Passport is still valid (i.e. not expired)
In order to apply for lost or stolen US Passport please follow the instructions:
STEP 1 - Gather US Passport Application Materials
- 2 copies of Letter of Authorization (2 copies)
- Letter of Authorization Form(age 18 and older)
- Letter of Authorization Form(age 17 and younger)
- Order Form
- Two Passport Photos
- Proof of US Citizenship
Please provide one of the following:
- Official government-issued Birth Certificate (if born in the US)
- FS-240, DS-1350, or Certificate of Citizenship (if born abroad to US parent(s))
- Naturalization Papers (for US citizens through naturalization)
- Proof of Identity
- A Photocopy of the applicant's Valid Driver's License, issued more than 6 months ago.
- Four filled out DS-11 & DS-64 Government Forms (two copies of each form)
FORM DS-11(click for form) FORM DS-64(click for form)- You MUST fill out this form electronically and then print 2 copies.
- Fill out all of the fields on the form.
- Click on the "create form" button to generate the completed form.
- Do not sign the form until you appear in front of your local passport Acceptance Agent.
- Confirm that the "Mail Passport To" field on the government form is completed with the actual address where you want the completed passport FedExed (no PO boxes).
- The government can insist that your completed passport be sent to the address that you have listed on your form, even if you give us a different address on your order form.
- The government can choose to send your passport back to you via regular mail if the addresses do not match.
- There is no way for us to verify the address that you have listed, thus we cannot be responsible for any delays associated with this restriction.
- Travel Evidence
Please include 2 copies of:- Copy of plane ticket showing foreign departure
- Copy of travel itinerary (from airline or travel agency with applicant name, travel date, flight info)
- If travel is for business and neither of above is available, see business travel letters
- Fees
- $30 collected by your local Acceptance Agent (explained below)
- $245 for 7 Business Day Processing, payment includes fee to Department of State, return shipping and handling, and our service fee.
- $395 for 24 Hour Passport Processing, payment includes fee to Department of State, return shipping and handling, and our service fee.
STEP 2 - Get lost/stolen US Passport Application Authorized
- All 1st Time or New Applicants need to have their applications authorized by local government officials called Acceptance Agents (AA).
Note: AAs are located in Post Offices and Courthouses around the country. Not every Post Office or Courthouse has an AA, and hours vary, so click here to find your closest one. The AA's job is to witness the applicant sign the government form, check their ID, and seal some of the application materials into an envelope. Please make sure the acceptance agent SIGNS your application, and stamps his/her round seal on the application as well. If this is not done correctly, it will delay the processing of your application.
The items that MUST be sealed are:- The signed DS-11.
- The original proof of citizenship.
- The two US passport photos.
- The second copy of the DS-11.
- The photocopy of the driver's license, or the supplemental ID.
- The 2 copies of the Letter of Authorization.
- The East Site Inc. Order Form.
- The travel evidence.
- The East Site Inc. payment.
- The AA should write the applicant's name, departure date, and date of birth on the outside of the sealed envelope. If two applications are sealed in the same envelope (normal), ensure that both applicant's information appears.
- The AA should attatch the second copy of the computer generated DS-11 to the outside of your sealed envelope.
- Once you have the sealed envelope, and the additional materials (as listed above), continue to step 3.
